Get Started With Your HavenPoint Veteran Referral

Your referral will be managed by our trained team, who are dedicated to protecting your personal information and ensuring complete confidentiality

To Help Us Get You Connected to Care as Efficiently as Possible

Please take a moment to gather the information listed below

    1. Visit: veterans.gc.ca/myVACaccount

    2. Choose how to sign in: Sign-In Partner (online banking) or GCKey (Government username & password).

      • If using GCKey, click Register and create your credentials.

    3. You may be asked for your VAC file number (CSDN) or service/regimental number.

    4. Complete any required security steps (two-factor authentication, security questions).

    5. Once complete, you will see your MyVAC dashboard, where you can:

      • Start applications

      • Upload documents

    1. Sign in to your MyVAC account.

    2. On the homepage, click “Benefits Navigator” or “What can I apply for?”

    3. Complete the short questionnaire to view your eligible benefits.

    4. Select the benefit you wish to apply for and open the online application (PEN923).

    5. For each condition, click “Add a Condition” on the form.

    6. Upload any required supporting documents (medical reports, service records, IDs).

    7. Review and submit your application.

    8. Monitor your MyVAC messages for updates or document requests.

    9. Once processed, Veterans Affairs will issue a letter confirming your approved conditions.

  • Please gather the following documents before contacting HavenPoint:

    1. Service or regimental number*
      Found on your service card, discharge papers, or military ID.

    2. VAC file number (CSDN) and K number (if available)*
      Shown on any VAC letter. Also visible in your MyVAC Profile.

    3. Summary of Assessment (SOA)*
      Located in your MyVAC Messages or Documents.

    4. Signed VAC928 consent form*
      Found in MyVAC under Forms (search “VAC928”).

    5. Any medical records or your doctor’s contact information

    6. Relevant diagnostic reports or test results

    7. Any VAC decision letters you’ve already received
